- Author: Who created it, or who made the content?[2] (if you are the author, enter your username or sign it with ~~).
- Date: when was the file created and if applicable when was it first published?
- Permission (only fill in if you are not the author, otherwise leave blank): How do you know that you are allowed to use the file at all? Has the author explicitly given you permission? Please forward the permission to permissions-de@wikimedia.org and don't forget to mention the file name!
